.a10 years girl presented with dehydration ,vomitting and abdominal pain for 10 days
her x_ray showed suspicious of intestinal obstruction as fluid levels but it were ordinary gases
fortunately, anurse noticed actone odour from her breath.
.she was managed butwhile therapy she arrested twice and now she is under ventilation in ICU.

mannitol is not contraindicated in brain edema while managing DKA .brain edema is apriority despite dehydration to prevent cardiac arrest and brain herniation.but dehydration should be managed too
normal K+ level before therapy indicates k+ supplementation as insulin therapy is associated with hypokalemia
to confirm DKA diagnosis urine analysis for ketone bodies is indicated as serum amylase may be elevated in DKA and also not indicator for intestinal obstruction.ECG may be normal.serum electrolytes not confirmatory.

انا اسفه جدا لانى مشاركتش معاكم من فتره ولانى ما وفدش بوعدى بانى اجيب حالات لان النت عندى كان فاصل لمده اسبوع والفتره دى كنت مشغوله بس هحاول باذن الله
بالنسبه للحاله دى انا لسه ساله الدكاتره فى الطوارىء النهارده على موضوعk+ supplementation on DKA with normal k+ levelانا لى نفس رايك بس هم قاللوى انهم ما بيعملوش كده لان insulin لما بينزل الkبيكون فى normal rangeوهم اكدولى ده فنا شايفه انى اخد برايهم مع الاحتياط انى اتابع +serum kلو نزل اديه وممكن انا فى الاول لو المريض فايق ويقدر ياكل اعطيه اغذيه غنى بالk+زى الموز مثلا
انا اسفه مره تانيه لانى ما وفدش بوعدى ووالله طلعت اللى كنت كتباها السنه اللى فاتت على امل انى ابص فيه وطلع حالات بس هحاول الفتره اللى جايه قبل دوشه سنه سادسه
